A mastectomy is an operation in which breast tissue is removed and the breast is given a masculine appearance. The goal is not only a flat chest, but also an aesthetic result that aligns with your wishes: the right proportions, an appropriate nipple size, and minimal scarring.
What does the operation involve?
During the procedure, we remove the breast tissue, excess skin, and the breast crease. Depending on the technique chosen, the nipples are adjusted and repositioned. The procedure is always performed under general anesthesia and is carried out by an experienced plastic surgeon.
Which technique suits you?
The choice of technique depends on breast size, skin elasticity, and personal preference. At Esthetisch Centrum Jan van Goyen, we offer the following techniques:
Keyhole
This minimally invasive technique is suitable for people with small breasts and good skin elasticity. Breast tissue is removed through a small incision along the underside of the areola, optionally supplemented with liposuction. The nipple stalk remains intact, so nerve sensation is largely preserved.
Peri-areolar technique (Donut)
With this technique, two concentric incisions are made around the areola. The skin between them is removed and joined using a purse-string suture. The nipple stalk remains intact, and sensation is often partially preserved. Because the inner incision is smaller than the outer one, some slight wrinkling may occur along the scar. Additional liposuction is an option.
Double Incision
A double incision is the most commonly used technique for people with medium to large breasts. Breast tissue is removed through two horizontal incisions, in the breast crease and along the areola. The nipples are removed, reshaped, and repositioned in the desired location as free grafts. After fixation, the grafts are covered for five to seven days with a dressing ('sponge') that promotes integration. After the transplantation, sensation in the nipple is limited; in rare cases, it is possible to preserve the nipple stalk.
Aftercare
After the procedure, you will stay with us for one night for observation. You usually leave the clinic the morning after the operation, wearing a compression bandage that you will wear day and night for four weeks. This helps prevent fluid accumulation and promotes the adhesion of the skin to the chest wall.
After the operation, you will have a drain on both sides. The drains will be removed after five days during an outpatient appointment. If desired, this can also be done by your own general practitioner, provided they are willing to do so.
For procedures involving nipple grafts, the dressings ('sponges') are also removed at the outpatient clinic after five to seven days. Until then, the dressings must not get wet.
Control appointments
We guide you from A to Z throughout the treatment process. After the procedure, you will have a number of follow-up appointments:
- Day 5: removal of drains; in case of double incision, also removal of nipple dressings
- After 2 to 3 weeks: wound check (with keyhole or donut, or without nipple graft)
- Six months after the operation: final check-up (on request)
